What You Need For Your First Visit

On your first visit you’ll need to complete some paperwork, which includes your contact information, medical history and billing information, before meeting with our medical team.

When you make your appointment, you can request a link to fill out the registration form online, before your appointment. If you complete your forms ahead of time, please arrive 30 minutes prior to your appointment.

You can also complete the forms in the office, before your appointment. If you would like to complete your forms in the office, please make sure to arrive one hour prior to your appointment to give yourself time to complete them.

As a new patient you will need the following documentation at your first appointment:

  • Proof of Address. (For example: Con Edison bill)
  • Insurance Card.
  • ID (For example: driver’s license, state ID, passport).
  • Immunization Card (for a child / pediatric visit)
  • Birth certificate (for a child / pediatric visit)
  • A referral or authorization from your primary care physician, if required by insurance. Please contact your primary care physician or insurance company if you have any questions.
  • The name, address and phone number of your primary doctor.
  • The name, address and phone number of your pharmacy.
  • Copayment amount designated by insurance carrier is expected at the time of service. Cash, check or credit card are accepted. Self-pay patients without insurance must bring tax returns from the previous year to determine sliding scale fee. Payment is expected at the time of service.

Please bring copies of any recent lab, x-ray, or imaging reports.

If the patient is under 18 years of age, a parent or legal guardian must accompany the patient. The legal guardian must bring proof of guardianship, picture ID and proof of address. If the minor is accompanied by someone other than a legal guardian, picture ID with a notarized letter signed by the legal guardian is required. If the patient is under 18 years of age and the patient has been declared an emancipated minor by US court of law, documentation must be provided.
